how to apply to the fund

Documents to be provided:

1. Original application manually(must be noted that parents do not object to placing information on the stand, the Internet and media).

2. Photo  child (ordinary landscape photo).

3. Information on family composition.

4. Statement of income.

Copies of documents:

1. medical certificate, statement prescription.

2. Copy of passport and copy of ID code parents.

3. Copy of the child’s birth certificate.

4. Accounts of medical institutions or bank details.

 All these documents must be sent by registered mail Ukrpochtoy to the Fund. The submitted documents will not be returned.

The presence of the above documents facilitates consideration of each appeal.
